WAIVER INFO:
1. TSR keeps waivers on file from Jan 1st to Dec 31st. This means you need to fill a new one out every year.
2. If you participate in both MotoCross and Wake, Skate, or Climb you will need to have a waiver at both checkin areas.
3. All Waivers not filled in at TSR in front of an employee must be notorized. (no exceptions)
4. Only a parent or legal guardian (Must have legal paperwork for guardians) may sign for minors.

Download the 2 page waiver, then fill out areas 1 through 4 after you have fully read and understand the form.
1 Fill out all pertinent information, make sure we have your emergency contact number
2 Initial the bottom of page 1 so we know that you have fully read and understand the document.
3 Sign here, if you are under 18 have your parent or guardian sign below. This needs to be done in front of a TSR employee or Notary. If you are signing it in front of a notary proceed to step four.
4 Have the notary sign and stamp the form to validate the signatures.
